Official Notification - (Use of MPSF Incident Report Form - per MPSF Water Polo Officiating Guidelines)
The MPSF Coordinator of Officials and Executive Director are notified via email upon submission of this online form.
Your  IP address is recorded when you submit, and severe legal repercussions shall arise in the event that this is not a legitimate report.

Use this form for:
1) Removal of a coach, player or bench personnel from a game, using a red card.
2) Unsportsmanlike conduct by a player, resulting in the removal from a game.
3) Brutality (not to include exclusions for over-aggressive fouls).
4) Other unusual or extraordinary situations
5) Conduct problems with coaches, players, bench personnel, desk/scoretable personnel, referees, goal judges, spectators, etc.
6) Significant incompetence by desk/scoretable  personnel/event management staff/practices.
Note: Game related facilities issues/desk incompetence or spectator problems shall be directed to host institution game managers during or immediately after games - do not report these types of problems to desk personnel.

DESCRIBE THE NATURE OF THE INCIDENT, INCLUDING: name(s) of individual(s) involved, injurie(s) sustained, action(s) taken, penalty assessed, and any other circumstances or considerations that had or may have a bearing on the situation. Please note any witnesses who can be contacted if necessary.
